Drop it off at our front door in a clear plastic box or mail it at Sabin CDC’s office at 1488 NE Alberta St. Portland, OR 97211. The application will be reviewed on a first come first serve basis after the City of Portland 48-hour waiting period.
The head of the household’s ID is required with the pre-application.
